Please indicate the names and instruments of the ensemble members performing in this group. List each member on a new line and separate each member name listing with a semi-colon. Example:

Please select any and all dates your group is available to perform. Please keep in mind the location of the performance venue when selecting a date as three concerts are at off-site locations and three concerts are at Symphony Center. Note that for concerts at Symphony Center, Armour Stage, performers will be faced upstage, with audience members sitting on the stage and in the terrace.

Information on community venues as follows:
Epiphany Center for the Arts, seats approx. 400 ppl.
North Park University seats approx. 400 ppl.
South Shore Cultural Center, seats approx. 400 ppl.

We request that each program contain a work that is aligned with repertoire and themes featured on the 2025/26 CSO Main Series Programs; particularly the celebration of America 250. Please list works in performance order and indicate the intermission.

The total duration of the music should be no more than 60 minutes.
